[Kc] list etiquette

Let's add a "list etiquette" section to our web pages.  Not that we
want to maintain
some kind of discipline WRT top/bottom posting and me-toos, but a
caution against
including an entire digest in reply, when someone is subscribed to the
digest feed,
might be appropriate.

Someone did this recently on kc at mail.pm.org and I apparently failed to
be sufficiently
gentle in pointing out in off-list communications that including an
entire digest is bad form.
