The reason you are getting back a message about an empty 
document is quite simple.  All you are returning is the content 
header.  You aren't returning anything to actually display!

Decide on what html you want to be returned when this CGI script 
is invoked, and emit that.  The easiest way is through a "here" doc, 
e.g., by adding the following to the end of your script:

print <<END;
<html>
<head>
<title>Thank you!</title>
</head>
<body>
<h1 align="center">Thank you!</h1>
<p>Your form has been successfully submitted.</p>
</body>
</html>
END
